the Hand

 

The exemple of affection often met in consultation : the disease of dupuytren. Retraction of aponevrotic tissue of the hand. Anatomical structures such as the tendons which make one's way in the hand are separated by partitions. Those which are between the flexor tendons and skin (which fold the fingers) retract and form true cords in the palm of the hand moving towards the fingers.

 

Symptoms :

The fingers are put in bending and cannot rectify itself.

Diagnosis :


The clinical examination. No examination is necessary. We measure the number of degrees of deficit of extension of the finger.

Treatment :


There is no medicamentous treatment.

The surgery can be discussed in the cases deficit of extension significant, awkward for the everyday life gesture.

Principle :


To remove the aponevrotic cord doing an incision in front of this cord, by taking care to release it compared with the vessels and the sensory nerves. At the end of the intervention, we obtain a complete extension of the finger.

Advantages :


We can move and use the fingers after the intervention.

Limits :


In certain evolved forms, a graft of skin can be necessary.

A clinical example :


The ring finger is touched ; it is in inflection with 110°
(either beyond the right angle) and cannot rectify itself.


At the end of the intervention,
the finger can be put in extension.


In this example, a graft of skin was carried out.


On this sight, the operated finger
is parallel to the different one.
